What is a keloid scar?

A keloid is the result of the body producing excessive scar tissue that goes beyond the original edges of the wound. In many cases, keloids become itchy, painful, and disfiguring. Keloids are very common on the ear after ear piercings, particularly piercings through cartilage. Even after the earring is removed and the piercing itself heals, keloids can continue to enlarge and proliferate. A keloid can also form in the head and neck along surgical incisions or after face, neck, or scalp trauma.

Keloid scars are unsightly scars that can arise from minor trauma to the skin and enlarge over time. These are often seen in the head and neck area and can be very bothersome for many people.

Keloid removal, or keloid excision, usually requires both surgical and non-surgical management. In some cases, keloids can become very large, and these large keloids will require a combination of treatments to achieve the best result. Many keloids require surgical excision, followed by other treatments like steroid injections. Patients need to follow up closely with their surgeon after keloid removal.

As facial plastic surgeons who specialize in facial aesthetic procedures, Dr. Richardson and Dr. Cain are well-equipped to remove even very large or disfiguring keloids and manage the area post-operatively with close clinical follow-up.

Is keloid scar removal safe?

Yes, keloid scar removal is a safe procedure that can be performed under local anesthesia. The risks of keloid removal include general risks of any procedure, such as pain, bleeding, and infection, but these risks are minimal. Excision of keloid scars can also cause cosmetic changes in the area from where they are removed. Because of this, it is important to have these removed by a surgeon who is knowledgeable and skilled in facial plastic surgery when these scars involve the head and neck.

An important risk to consider with keloid removal is the risk that a keloid scar will return after surgery. This is because keloid scars arise after trauma to the skin, and surgical removal of the scar is a form of trauma itself. However, this is a very controlled type of trauma, and when combined with other modalities to prevent keloid recurrence, such as steroid injections or radiation therapy, the risk of recurrence can be minimized.

What happens during keloid removal?

Keloid scars are most often removed under local anesthesia. An incision is made through the skin overlying the keloid scar, and the scar tissue is dissected out from underneath the skin and removed. Excess skin that has been stretched out by the enlarging scar is also removed so that the wound can be closed with a flat scar that is not under tension.

Sometimes, a steroid may be injected to the surgical site during the procedure to help prevent keloid recurrence during the healing period. Sutures are used to close the wound, and these are usually removed about one week after surgery.

Is keloid scar removal painful?

No, keloid scar removal is usually performed after an anesthetic has been injected to make the area numb. The surgical site may be a little tender for a day or two after the procedure, but this is usually easily controlled with over-the-counter medications.

Am I a good candidate for keloid scar removal?

If you have a keloid scar that is unsightly or causing discomfort or other symptoms, you may be a good candidate for keloid scar removal. Dr. Cain or Dr. Richardson will evaluate the keloid scar during your consultation and discuss options for treatment with you, which may include non-surgical or surgical options, or a combination of both.

How long does it take to see results after keloid scar removal?

You will see the results of keloid scar removal immediately after surgery, although it can take some time to completely heal. There will be a much smaller scar where the keloid was removed, but with appropriate follow-up care this should stay small and flat, and will eventually fade to be less noticeable over time.

What is downtime and recovery like after keloid scar removal?

After the keloid scar is removed, you will have some sutures in place that are usually removed about a week after surgery. During that time, you are asked to keep the area clean and avoid any strenuous activity or heavy lifting. However, you should be able to return to most normal daily activities such as work or school immediately, even the same day as your surgery. At 1-2 weeks after surgery, most patients are able to return to all activities including exercise.

Is keloid scar removal permanent?

Keloid scar removal is often permanent, but there is an inherent risk of recurrence when dealing with keloid scars. In order to minimize this risk, surgical excision is typically combined with non-surgical treatment modalities that may be performed before, during, and/or after your surgery, such as steroid injections or radiation treatments.

Who is qualified to perform keloid scar removal?

Excision of keloid scars from the ears, face, neck, and scalp should be performed by a plastic surgeon with expertise in facial aesthetics and experience in maintaining the appropriate form and function of the facial features affected by the keloid scar tissue. This surgeon should also have experience in combining both surgical and non-surgical treatment modalities to prevent recurrence of keloid scar tissue after surgery.

How much does keloid scar removal cost?

The cost for removal of a keloid scar varies widely based on the size and location of the scar and the complexity of the procedure. In general, the price will typically range from $2,500-3,500. This may include adjuvant treatments such as steroid injections for a period of time after surgery, but it does not include the cost of any therapy performed outside of our practice, such as radiation therapy.
